The Practice Nurse will arrange immunisations for travel; a fee may be payable for some vaccines.
Please complete and return our travel questionnaire at least 6 weeks before you travel so that our nurse can give you advice as to what treatment is appropriate depending upon the country and area you are planning to visit.
As some of the vaccinations are given in courses of 3 and take time to be effective it is better to return the form as soon as possible.
